Copyright Notice Procedure

Our Copyright Notice Procedure ensures that all disputes are handled in a manner that respects both the copyright holder's rights and the provision of free and lawful information.

Submitting a Copyright Infringement Notice

If you believe that your copyrighted work has been posted on Forms Georgia without your permission, please send a notification of claimed infringement to our designated agent at dmca@formsgeorgia.com. To enable a swift and accurate review process, your notice must include the following information:

  • A physical or electronic signature of the person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
  • Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works are covered by a single notification, a representative list of such works.
  • Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit the service provider to locate the material.
  • Information reasonably sufficient to permit the service provider to contact the complaining party, such as an address, telephone number, and, if available, an electronic mail address at which the complaining party may be contacted.
  • A statement that the complaining party has a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
  • A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Upon Receipt of a Copyright Infringement Notice

Upon receipt of a properly constituted infringement notice, Forms Georgia will follow the following steps:

  • Respond to the infringement notice promptly.
  • Conduct an initial assessment of the complaint to determine its validity.
  • If the complaint is found to be valid, the allegedly infringing material will be removed or access to it will be disabled.
  • Notify the content provider, member, or user that the material has been removed or access to it has been disabled.

Forms Georgia respects copyright law and adheres strictly to the legal requirements set out by copyright legislation. In safeguarding the rights of copyright owners, the service also ensures that the free flow of information and legal material is not unduly hindered. The aforementioned procedures have been instituted with the intent to strike a balance between upholding copyright and promoting access to informational resources.
